The Healthy Aging & Memory Clinic is one of the Midwest’s leading primary care geriatric centers, serving thousands of patients each year with a multidisciplinary approach to evaluation and treatment. The staff includes a social worker who assists families with identifying and accessing community resources as they plan for the future.
